The role of the Partners in Advanced Care Team (PACT) Bereavement Coordinator is to support Nemours' grieving families. Bereavement support will include partnership with PACT psychosocial partners such as the palliative care social worker, child life specialist , other hospital clinicians, community clinicians, and resources as needed. The job includes phone support, assessing family resources and facilitating resource linkage, and follow-up check-ins with bereaved families, referring to a higher level of care or other licensed mental health clinician when needed. The Bereavement Coordinator will maintain appropriate electronic medical record (EMR) documentation and plan for relationship termination from a place of growth and healing.
Additional responsibilities include partnering with PACT psychosocial disciplines to facilitate / cofacilitate caregiver support groups and participate in ongoing bereavement programming and program development, including but not limited to the annual Remembrance Ceremony and Camp Molly. It will be expected that the bereavement coordinator identifies opportunities for grief education within Nemours and facilitates community-based connections with local agencies. Opportunities to participate in psychosocial research will be present but are not required.
Bachelor's Degree Required
LPC, LCSW, MSW, or MDiv, Chaplaincy board certification (such as BCC), pastoral counseling certification or other related to the role required
Minimum 3 years of experience as a psychosocial practitioner; bereavement experience preferred
