Gurwin Healthcare System is a full continuum of care providing extraordinary care and caring to the people that we serve. Set on a 36-acre campus in Commack, NY, the Gurwin Healthcare System includes the 460-bed Gurwin Jewish Nursing & Rehabilitation Center, providing short-term rehabilitation and long-term skilled nursing care, memory care, respiratory and ventilator care, palliative and hospice care, social and medical adult day programs, and on-site dialysis. Sharing the campus is Gurwin's Assisted Living community (Gurwin Jewish ~ Fay J. Lindner Residences), our independent living community (Fountaingate Gardens) and our Gurwin Certified and Licensed Home Care Agencies.
Our Adult Day Care Program is seeking a full-time LPN who will be scheduled to work from 8:30am-4:30pm with a weekend rotation.
EDUCATION AND EXPERIENCE REQUIREMENTS:
Graduate of an accredited LPN program
Licensed by the New York State Education Department as an LPN
Previous work experience in Long Term Care (recent ADHC experience preferred.
PRINCIPLE FUNCTIONS:
Assumes primary nursing responsibility for the the health management of the registrants under the direction of an RN.
Coordinates the holistic care management of the registrants with the family/caregiver, Licensed Practitioner and Interdisciplinary Team under the direction of an RN.
DUTIES AND RESPONSIBILITIES:
Identifies registrant health needs and provides for appropriate hands-on care and monitoring under the direction of an RN.
Assist the RN in the day-to-day functioning of their designated Nursing Station.
Directs, supervises and instructs CNAs in the clinical aspect fo the Program.
Cooperates with other desciplines on coordinating direct and indirect services to registrants and families/caregivers.
Supervises registrants medication regime, administers medications and maintains medication administration record as needed.
Various other duties.
