Job Description
Under supervision, meets or communicates by telephone, e-mail, or fax with key referral sources in a consultative manner regarding their home care needs and present appropriate solutions. Aids the paper flow of new referrals to ensure timely provision of services.
Job Responsibility
+ Assists in processing referrals for one or more home care product lines.
+ Aids the completion of enrollment documentation.
+ Initiates insurance verification form.
+ Keeps referral logs on daily basis.
+ Maintains appropriate files and records.
+ Maintains current knowledge of regulatory, reimbursement and third party contract guidelines.
+ Documents pertinent referral information within established timeframes and payer guidelines.
+ Performs related duties as required. All responsibilities noted here are considered essential functions of the job under the Americans with Disabilities Act. Duties not mentioned here, but considered related are not essential functions.
Job Qualification
+ Vocational or Technical school Diploma required, or equivalent combination of education and related experience.
+ Current License to practice as a Practical Nurse in New York State, and Current Basic Life Support Professional Certification required, plus specialized certifications as needed.

The salary range for this position is $51,830-$79,300/year
