**PruittHealth@ Home in Cumming, GA services home health patients in their private homes and assisted living facilities throughout Cherokee, Forsyth, and North Fulton Counites as well as Buford, Sugar Hill and the one Lawrenceville zip code that merges with Buford, GA. We are seeking LPNs that live or want to work throughout any of the mentioned areas. Home Health experience is not required. + Complete evaluations, assessments, and treatments in accordance with policy and the individualized care plan

+ Complete all documentation and records in a timely and accurate manner

+ Follow established policies and procedures as related to the scope of practice

+ Maintain continuity of patient care in a home care setting

**LICENSURE, CERTIFICATION, AND EDUCATIONAL REQUIREMENTS**
