Nurse Practitioner, Lifespan Bleeding Disorders Program
The London Health Sciences Centre (LHSC) Lifespan Bleeding Disorders Program (BDP) is a regional program that provides comprehensive care for children and adults with inherited and acquired bleeding disorders and their families. It is a program serving the people of Southwestern Ontario since 1978 with a catchment area of 2.6 million people.
The Nurse Practitioner is a self-directed advanced practice nurse prepared at the graduate and post-graduate level, who demonstrates a high level of autonomy working to scope, both independently and within a collaborative practice model. Using advanced assessment skills and a sound knowledge of clinical therapeutics, the NP demonstrates a comprehensive knowledge base in and exhibits a high degree of clinical competence in caring children and adults within the Bleeding Disorders Program.
The NP will report to the Director of Paediatric Inpatients and Ambulatory Care and will work collaboratively with members of the Lifespan Bleeding Disorders Program. The NP will interact daily with the interdisciplinary team to manage, direct and provide comprehensive care for a select patient population. The NP works with key stakeholders to provide advanced care that optimizes patient outcomes and enhances safety, effectiveness and accessibility. The NP in this program will engage in scholarly activities and advance knowledge by providing clinical leadership, identify opportunities for improving patient care, advancing best practices, promoting professional education and engaging in research.
With a focus on service excellence and patient safety, principal responsibilities include:
- Clinical leadership: First point of contact for patients with bleeding disorders, as well as hospital and community partners
- Program Leadership: Overall coordination and functioning of the BDP Outpatient NP Led clinics seeing both new consults (within scope) and follow up patients
- Ongoing education, counselling and advice to patients/families and other care providers. Includes prescribing factor replacement and other relevant medications, ordering diagnostic imaging and making specialist referrals
- Focus on outpatient management to prevent acute bleeding complications which reduces associated health care resources such as visits to the Emergency Room (ER) and inpatient hospital services
- Educate, support, and provide oversight to patients eligible for the Home Infusion Program (HIP). Certification of patients on the HIP creates autonomy in skill self-managing a lifelong chronic health condition
- Support patients/families and other care providers in coordination and management of care around invasive procedures (dental/diagnostic/surgical)
- See patients independently outside of regular BDP clinics and liaise with BDP Medical Directors as needed, for advice and counselling
- In consultation with the Adult and Pediatric Medical Directors, follow up on inpatients (surgical or acute bleeds) where appropriate
- Provide Leadership as Co-Chair of the Bleeding Disorders Program Advisory meetings, Chair monthly BDP team meetings, Chair monthly meeting with Hemostasis and Thrombosis Laboratory
- Mentor and support interdisciplinary BDP team members
- Develop/update BDP standard operating procedures/policies/Power Plans for efficiency and optimal patient care that reflect the Canadian Integrated and Comprehensive Care Standards for Inherited Bleeding Disorders (June 2020) and the World Federation of Hemophilia (WFH) Guidelines for the Management of Hemophilia, 3rd edition
- Identify opportunities for quality improvement
- Provide in-service educational sessions to other areas of the hospital
- Participate in clinical research with other BDP team members
- Attend relevant Bleeding Disorders conferences and professional meetings [e.g., AHCDC (Association of Hemophilia Clinic Directors of Canada), CANHC (Canadian Association of Nurses in Hemophilia Care), WFH (World Federation of Hemophilia) and, ISTH (International Society of Thrombosis and Hemostasis)]
- Participate in clinical, educational, administrative and research activities of CANHC and other Hemophilia organizations, CHS (Canadian Hemophilia Society) and the WFH including hemophilia summer camps

Qualifications
- Successful completion of a Masters Degree in Nursing
- Current registration in the extended class in any of the following specialties: Primary Care or Paediatric, and in good standing with the College of Nurses of Ontario
- At least 3-5 years nursing experience with clinical focus in Bleeding Disorders and/or Hematology
- Current member of the Nurse Practitioner's Association of Ontario (NPAO) or Registered Nurses Association of Ontario (RNAO)
- Mandatory Current Professional Liability Protection
- Current Basic Life Support for Healthcare Providers course: BLS-HCP (C)
- Demonstrated experience and leadership in clinical practice, education, program development and research
- Expertise in functioning in an inter-disciplinary health care environment
- Ability to establish effective interpersonal relationships with colleagues and therapeutic relationships with patients and families
- Expertise in critical thinking, clinical problem solving, negotiation and conflict resolution
- Effective organizational, conceptual and leadership skills
- Ability to provide project management skills to promote evidence-based strategies and initiatives and ability to work in partnership with relevant LHSC staff and physicians
- Experience in/or understanding research, program development and evaluation
- Demonstrated excellence in interpersonal relations and communication skills, both oral and written
- Demonstrated excellence as a role model for clinical leadership
- Demonstrated experience with clinical teaching in an academic environment
- Advanced presentation skills including the application of adult learning principles, change theory and systems theory in a clinical environment
