Compensation Range:
Anticipated Base compensation rate is in the range of $63.14 - $63.14 / Hour depending on qualifications and experience.

Expectations:
Under the supervision of the Supervisor, Community Nursing and with direction from the program's Clinical Supervisor, provides health-focused assessment and care planning services to adults with disabilities and older adults and their families in various programs. Provides services in home and community-based settings to preserve health, maintain function and independence through assessment of needs, monitoring quality of care, and providing education, advocacy and support. Provides nursing consultation and guidance to departmental staff. This position requires flexibility of hours.
EDUCATION:Graduate from accredited nursing or higher preferred. Diploma or AA degree: Effective for hires on or after 9/14/15, must be currently enrolled or accepted to a BSN or MSN program within one year of employment; degree must be completed within 3 years. Diploma or transcripts on highest degree earned required.
EXPERIENCE/TRAINING:Minimum clinical nursing experience with adults for at least 10,000 hours (5 years full-time or equivalent) in an acute care hospital or 2,000 hours (1 year full-time or equivalent) in a home health setting.
LICENSES/CERTIFICATIONS:Required:Current unrestricted Registered Nurse (RN) license issued by California Board of Registered Nursing (BRN)Current Basic Life Support Provider (BLS) issued by American Heart Association
SKILLS:Clinical assessment skills, knowledge of community resources and public benefits, ability to work with a multidisciplinary team and with clients/families in the home setting. Knowledge of aging programs for seniors and/or adults with disabilities. Computer skills including Microsoft Office and ability to learn and utilize specialized software with training and support. Ability to provide nursing consultation and guidance to departmental staff.
