• Provision of comprehensive, coordinated health care encompassing primary care, illness prevention and health promotion • Manage a roster of a maximum of 1137 primary care clients • Working in a collaborative environment with a diverse interprofessional team of other physicians, and nurse practitioners • Collaborate with our team of professionals including social worker, dietitian, pharmacist, respiratory therapist, diabetes educators, chiropodist, nursing, and community health workers • Shared on-call services amongst primary care provider team • Incorporating a health promotion approach, addressing the social determinants of health
