For Water Street Mission to fulfill its mission and purpose, it is critical for all staff to participate in the development and maintenance of a healthy Christian Community. Job Summary: Provide nurse care management services for Water Street Mission guests. Perform nursing and clinical administrative duties in collaboration with other health professionals to ensure safe and efficient patient care
FLSA: Full Time (32 hours), Non-exempt

Essential Functions:
Culture and Values

- Walk alongside fellow WSM staff in Christian community as we journey toward maturity.
- Encourage restoration in the lives of coworkers and guests, recognizing we are each restored by God to become restorers.
- Work in full coordination with other WSM staff at all times to identify, report and respond appropriately to any potential threats to the safety and wellbeing of guests.
Role Specific Responsibilities
Essential Clinic Duties:
- Focused nursing triage and assessment.
- Perform nursing procedures including phlebotomy and point of care lab tests.
- Administer medications, immunizations, and PPD tests.
- Clinical administrative duties related to, but not limited to EPIC work queues, quality assurance initiative, peer review process, chart reviews, and policy & procedure updates.
- Oversee and ensure proper process of medication fills form providers' orders in the pharmacy.
- Room patients for the providers.
- Accurate and complete chart documentation and form completion.
- Evaluate and meet patient social needs with available resources.
- Provide charge nurse oversight and ensure efficient flow of clinic sessions.
- Provide orientation and training for volunteer nurses.
- Supervise volunteer nurses during clinic sessions.
- Assist providers with scribing as needed.
- Teach or coordinate health related classes as assigned.
Essential Case Management Duties:
- One on one consultation with guests during Providence Day shelter hours.
- Assessment and triage of medical issues for Providence shelter guests.
- Provide nurse care management services for guests and patients with multiple, chronic medical issues.
- Make referrals to community health services and resources.
- Accurate and complete chart documentation and form completion.
- Attend medical appointments with guests as needed.
- Collaborate with WSM program staff on guest's health related issues.
- Attend appropriate multidisciplinary team meetings.
- Other duties pertaining to the essential functions.
Education and Experience: An Associate's degree accepted a Bachelor's degree preferred and a minimum of 2 years' experience in the nursing field. Unrestricted License to practice nursing in Pennsylvania. Experience working with patients that have a history of trauma.
Reports To: Director of Medical Services
Key Internal Relationships: Medical staff and volunteers, dental staff and volunteers, Access Center, Case management, Mission Directors, Key community medical providers
