Take Over a FHO-FHT Practice in Stoney Creek

A physician with a solo FHO/FHT practice in downtown Stoney Creek would like to retire and is looking for a successor to take over the care of his patients (start date flexible). The practice is currently in the early planning stages of co-locating this practice with other FHO colleagues. This is an excellent opportunity to take over an established, well-managed FHT practice in a team-based setting.

The successful candidate will be joining a large 26 member FHO and will care for a roster of approximately 1,500 patients (with an additional 250 un-rostered patients). With appointments typically booked at 15 or 30 minute intervals depending on the complexity of the patients, the practice typically sees between 30-35 patients per day (including patients seen by the nurse). Current practice hours are 9 a.m. - 5 p.m. on Monday, Tuesday, and Thursday, and half-days on Wednesday and Friday. On-call coverage is provided to the FHO 1-2 evenings per month and 4-6 weekend day shifts per year (a shift is comprised of 3 hours of in office coverage). Up to 6 weeks of vacation coverage is provided by the FHO each year.

This solo practice is located in a large medical building in downtown Stoney Creek alongside a lab, x-ray/imaging clinic, and 5 FHO colleagues. The current practice space uses Telus PS Suite EMR and includes a reception area/waiting room, 3 exam rooms, a private office, a counsellor suite, washrooms, and free parking. Experienced administrative staff handle reception and patient bookings, and as part of the Hamilton Family Health Team the practice is also supported by a full-time registered nurse, a dietician, and part-time mental health counsellor. A HFHT psychiatrist is also available for consults.
