As a key member of the Gateway Community Health Centre Team, and an integral part of the inter-professional health care team, the Primary Care Pharmacist delivers pharmaceutical care in accordance with policies and professional practice standards. Improves client outcomes by optimizing drug therapy, conducting client assessments for medication interactions/adverse effects, and provides health teaching and education focused on client centered objectives.
This position is available as a permanent part-time or full-time position.
Service Delivery Responsibilities
- Client Care
- Perform Client Medication Interviews & Assessments
- Conduct medication histories, identify drug-related problems, develop, and monitor care plans and communicate the plan to the client and inter-professional team on optimal use of medications (e.g., proper use, avoidance of overmedication, advise on non-prescription or over-the-counter drugs).
- Monitor medication adherence for specific clients/medications e.g., polypharmacy and/or addictive substances.
- Makes decisions regarding client care within the scope of practice of Registered Pharmacist, including but not limited to, client medications, clinical processes for prescription renewals, minor ailment prescribing, communication of product alerts, warnings, and advisories.
- Inter-professional Education
- Consult with the inter-professional team to assist with individual client care.
- Act as a resource to the inter-professional team on therapeutic topics or various aspects of the pharmacist practice or other medication-related procedures.
- Perform Seamless Care Activities
- Provide communication to hospital, specialists, community pharmacists, and other interdisciplinary team members as required to ensure a smooth transition for medication-related care between care providers.
- Develop Policies & Procedures to Improve Medication Use
- Develop medication use policies and procedures (e.g., processes related to drug prescribing, handling of samples, documentation, prescription renewals).
- Review existing medication use policies and procedures and recommend changes to keep current and improve client care.
- Other Activities
- Involvement in regional primary care pharmacy groups.
Skills, Knowledge Requirements
- Compliance with the pharmacist practice standards used by the Ontario College of Pharmacists, the Faculty of Pharmacy at the University of Toronto, and the National Association of Pharmacy Regulating Authorities.
- Strong clinical skills.
- Ability to use key medical and pharmacy literature databases and critically appraise primary literature.
- Excellent oral/written skills, and ability to work collaboratively with members of the health care team.
- Autonomous and able to function with minimal supervision.
- Ability to prioritize, manage time effectively, and be flexible in a very active work environment.
- Ability to work within a PC office environment, using an Electronic Medical Record.
Experience, Education, and Specific Job Requirements
- Pharm D or Baccalaureate Degree in Pharmacy.
- A minimum of two (2) years’ clinical experience as a Pharmacist, preferably in community-based health care.
- Must have a current Certificate of Registration in good standing with the Ontario College of Pharmacists.
- Community or hospital pharmacy residency completion or equivalent clinical experience.
- Additional training or credentialing in pharmacotherapy (e.g., Certified Geriatric Pharmacist, Certified Diabetes Educator, Certified Asthma educator, etc.) considered an asset.
- Additional training such as the Structured Practical Experience Program Teaching Associate Educational Program (University of Toronto) and in Evidence-Based Medicine Principles considered an asset.
- Completion of or willingness to complete the minor ailment prescribing modules.
Compensation Package
Gateway CHC offers a compensation package that includes a comprehensive benefits package, including Healthcare of Ontario Pension Plan (HOOPP) and four (4) weeks of vacation time, pro-rated in accordance with FTE status.
