Job purpose
The Nurse Manager works collaboratively with providers and support staff to ensure the smooth and efficient operations of the department in order to provide quality patient care and meet the goals and objectives established by the Health Center.
Duties and responsibilities
The Nurse Manager is responsible for directing, organizing and supervising the work of their nursing staff, which typically includes registered nurses (RNs), licensed practical nurses (LPNs), and Medical Assistants (MAs). They also coordinate nursing efforts to ensure that effective patient care is being provided and that quality standards are met. A clinical nurse coordinator is considered a middle manager, and reports to a nursing administrator, and follow Fair Haven Community Health Care (FHCHC) policies and mission.
Typical tasks include but are not limited to:
+ Provides primary clinical operation oversight, including scheduling, billing functions and patient care; and supervision of staff consistent with department goals, personnel policies and individual job descriptions
+ Demonstrates leadership and strong clinical support to department staff
+ Supervises RN, LPN and medical support staff
+ Develops and monitors employee performance plans; monitors and evaluates nursing and support staff activity and takes corrective action where needed
+ Assists in the recruitment, development, orientation and training of nursing and clinical staff
+ Serves as a clinical leader in various projects and forums
+ Ensures integration of nursing services with other patient care services in the facility and ensures nursing staff patterns are implemented
+ Ensures smooth and efficient processing of patients in the department; ensures established quality assurance, Joint Commission, UDS standards and infection control activities are performed according to policy
+ Responsible for data collection systems, fulfilling grant requirements and reporting statistical data necessary for grant purposes
+ Attends mandatory supervisory nursing meetings and occasional weekend trainings as required.
+ Performs assessments and data collection in an ongoing and systemic manner, focusing on quality metrics
+ Engage in performance improvement activities and act as an agent of positive change by looking for innovative ways to improve processes and advance clinical excellence
+ Provides nursing care services as needed, including but not limited to administering medications, assessment, planning, treatment and evaluation of patient care. Documentation of patient responses to treatment and care provided in the medical record, per FHCHC guidelines.
+ Positively represent FHCHC to internal and external clients
+ Serves on at least one committee.
+ Perform other necessary duties as required by FHCHC to achieve the goal of providing excellent primary health care in a Patient Centered Medical Home.
Qualifications
+ Connecticut Registered Nurse certification required (BSN preferred), a combination of education and experience may be substituted for the degree requirement. CPR certification from the American Heart Association required. Excellent leadership skills including the ability to motivate & supervise others; the ability to act discreetly and with diplomacy in relating to others, both internal and external to the organization, in personnel management and patient care services is also required.
+ Knowledge and experience with primary care setting, including program development and quality assurance methodology; familiarity with PDSA and Lean methodology is highly preferred. A minimum 2 years of ambulatory care preferably in an FQHC setting and 1 year of supervisory experience in an ambulatory setting is required. The ideal candidate will have demonstrated ability and excellent skills to independently carry out assignments and nursing tasks; proficiency with Microsoft Word, Microsoft Excel, patient registries, and web-based tools and the use of an EHR; strong knowledge of medical terminology and relevant procedures; excellent written, electronic and oral communication skills; and strong critical thinking and problem solving skills.
+ The ability to work effectively in a team environment is essential. Oral and written proficiency in English is a must.
+ Bi-lingual with fluency in Spanish preferred but not required.
