**This position will be working in Cully Assisted Living.**
**Schedule** : Two (2) weekdays week on day shift
**Job Description:**
Under the direction of a Registered Nurse the LPN provides quality nursing care in a clinic or community setting. All duties are to be performed within the Standards and Scope of practice for the LPN. Functions within the Interdisciplinary Team (IDT) to formulate and evaluate the Participant's Plan of Care (POC). Completes focused assessments, by collecting both subjective and objective data. Assesses educational needs and develops trainings for Participants, families and caregivers. Identifies areas of need or potential change in a Participant's status. Performs clinical tests, treatments, therapies, medication administration, health teaching and counseling, as directed by the PCP and RN.
Providence PACE is a Program of All-Inclusive Care for the Elderly that strives to keep older adults as healthy as possible living in the community through clinics, home visits and more. **Required Qualifications:**
+ Education to meet certification, license or registration requirement
+ Upon hire: Oregon Licensed Practical Nurse
+ Within 30 days of hire: National Provider BLS - American Heart Association
**Preferred Qualifications:**
+ Experience in a clinic or clinic-like setting
