Position Summary:
Sea Mar Behavioral Health's diverse and quickly expanding program is seeking a Full-time Licensed Practical Nurse for its Vancouver Behavioral Health Program located at Rose Village (formerly known as Community Services NW). The community based LPN Will work out of Clark County only.
This is an LPN position working with a specialized behavioral health team (PACT). The Program for Assertive Community Treatment (PACT) is a person-centered recovery-oriented mental health service delivery model that has received substantial empirical support for reducing psychiatric hospitalizations, facilitating community living, and enhancing recovery for persons with serious mental illnesses. PACT is designed specifically for persons who have the most severe and persistent mental illnesses, who have severe symptoms and impairments, and who have not benefited from traditional outpatient programs. The PACT team consists of Mental Health Professionals, Case Managers, Peer Support Specialists, RNs, ARNPs, and Administrative Support working together to provide comprehensive behavioral health care.
The LPN is responsible for providing general nursing care to all patients as ordered by the patient's provider and/or those procedures covered by current standing orders. The LPN may assist in the management of patient flow issues, patient acute illness screening, rooming patients, assisting providers in procedures and phone messages. The LPN will be required to follow other instructions and to perform other duties as assigned by his\her supervisor. Team oriented. Works collaboratively with all other healthcare team members to provide the best outcomes for the patients and the organization. The LPN shall focus on, lead and create excellent outcomes for the Clinical Quality Measures. Must be able to support a provider (MD) who sees an average of 21 patients per day or a provider (ARNP/PA) who sees an average of 18 patients per day.
Qualifications
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.
+ The person in this position must be of good character and be able to maintain the understanding, physical health, emotional stability and personality suited to meet the physical, mental, emotional, and social needs of the population served.
+ Excellent patient assessment skills are necessary.
+ He/She must possess interest and the ability to works with patient groups across the life span.
+ An understanding of managed health care is desired.
+ Excellent customer service skills are required.
+ Must be able to support a provider (MD) who sees an average of 21 patients per day or a provider (ARNP/PA) who sees an average of 18 patients per day.
+ Must have graduated from an accredited college or university with an active LPN license in the state of Washington.
+ Ambulatory care experience highly preferred.
+ He/She must have the ability to read and write proficiently in English with excellent chart documentation skills.
+ Proficiency in both English and Spanish is preferred, and may be required based on ethnic population of the clinic.
+ He/She must possess good communication skills with variable types of people and situations and be able to interact sensitively and appropriately with people of diverse backgrounds.
+ He/She must be able to calculate medication dosages and do simple conversions e. g. lbs. to kgs.
+ He/She must have demonstrated ability to make decisions independently and to seek assistance when appropriate.
+ He/She must demonstrate a strong sense of medical priority.
+ He/She must have the ability to solve practical problems and a variety of instructions furnished in written, oral, diagram, or schedule form.
+ He/She must be able to anticipate the needs of the patient, provider and clinical unit according to presentation and available resource
Education and/or Experience:
+ The LPN must be licensed in the State of Washington to practice as a licensed practical nurse.
+ Must be qualified to administer injections/medications.
+ Previous ambulatory care and acute illness screening experience desired.
+ He/she must have experience working with range of patient age groups and health concerns.
+ Must be licensed to practice as an LPN in the state of Washington and state mandated HIV education.
+ This position must obtain Basic Life Support (BLS) CPR within 90 days of hire date and is required to maintain current BLS CPR throughout employment.
The LPN must sign an oath of permanent confidentiality covering all patient related information and must receive a background check from the Washington State Patrol. The employee must meet the organization's health screening requirements for tuberculosis and immunizations status.
