About Providence Health Care & Foundry
Opportunities are currently available for Nurse Practitioners at the Foundry Vancouver Granville clinic. Owned and operated by Providence Health Care, the clinic provides physical & sexual health, mental health, substance use, peer support and social services via a multi-disciplinary team to youth between the ages of 12 and 24.
Reporting to the clinic Director, the Nurse Practitioner provides episodic, enhanced/specialized and longitudinal care. The collaborative team includes physicians, nurse practitioners, registered nurses, social workers, counsellors, occupational/rehab services, peer support, and administrative staff. At FVG, Nurse Practitioners also have access to shared care consultation with the psychiatrists on site.
The position collaborates with patients/clients/residents and other health professionals to identify and assess trends and patterns that have implications for patients/clients/residents, families and communities; develops and implements population and evidence-based strategies to improve health and participates in policy-making activities that influence health services and practices. The position participates in peer review and self review to evaluate the outcome of services at the patient/client/resident, community and population level.
