About The Opportunity
Responsibilities include obtaining a best possible medication history (BPMH) for patients seen by the Renal Program. Conducting patient medication history interviews with patient and family, collecting patient information from the Drug Information System (or alternate source if not available), documenting BPMH in patient chart to be used to create admission medication orders, reconciling medication orders completed by another health care provider, communicating to staff and physicians regarding medication reconciliation, problem solving with the patient care team and pharmacy staff, medication reconciliation at all transitions of care. Medication reconciliation RPhT (PPA) acts as a resource to staff and physicians, directs team members to ask for BPMH completion, confirms accuracy of patient medication information with community care providers or patient’s medical record. Cross-training will also be provided to help support drug access navigation as required to support zonal needs.
About You
We would love to hear from you if you have the following:
Drug administration by injection technical permit (Pharmacy Technician) an asset. Experience as a pharmacy technician/pharmacy practice assistant, preferably in a hospital, an asset. Knowledge of sterile technique and special precautions used with chemotherapy are an asset. Experience in unit dose drug distribution systems, automation technology, decentralized medication dispensing cabinets, an asset. Knowledge of medical terminology, inventory control and record keeping. Demonstrated good decision making and problem solving skills. Demonstrated ability to perform mathematical calculations related to medications. Knowledge of the law and regulations pertaining to the receipt, storage and distribution of prescription and narcotic drugs. Basic typing skills or computer/keyboard experience; skills in Microsoft Office Suite, Excel, etc. Effective communication and interpersonal skills. Competencies in other languages an asset, French preferred. Please ensure your resume is up to date and includes all relevant education, experience, training, and certifications.
Hours
Long-Assignment / 100% FTE / 75 Hours Bi-weekly. Approximately 12 months.
Compensation and Incentives
Registered Technician: $24.96 - $27.89 Hourly (Pharmacy Practice Assistants: $23.19 - $26.27). Successful candidates may be eligible for our benefits package which includes health, dental, travel, long-term disability, and life insurance coverage as well as a defined benefit pension plan.
