Essential Functions:
+ Performs nursing care within the scope of education and ability providing direct nursing care to residents requiring skilled nursing care.
+ Completes admission assessment, and initiates care plans.
+ Administers medications and tube feedings in accordance with established policies and procedures. Is familiar with indication for use, action, and major side effects of medications administered.
+ Takes direct action in urgent and emergent situations.
+ Anticipates and acts to maintain a safe environment.
+ Implements the plan care for individual residents.
+ Supervises nursing assistant staff in direct resident care by:
+ Giving a verbal and written report at the beginning of each shift.
+ Observing CNA/resident interaction and completion of resident care including bathing, grooming, dressing, feeding, toileting, mobility and attendance of activities, and appointments.
+ Discussing a plan of care for individual resident.
+ Making rounds and receiving feedback on resident status at the end of the shift or at any time there is a change in a resident's condition.
+ Communicating information regarding job performance on an ongoing basis.
+ Evaluates, in collaboration with the Charge Nurse/Supervisor/DON, the job performance of CNAs.
+ Make rounds on residents at least three times each shift assessing for changes in condition and response to care with treatment.
+ Is knowledgeable of common health problems of older adults and usual treatment, incorporating that knowledge into daily practice.
+ Documents appropriate and necessary information in the medical record.
+ Monitor the status of residents with acute problems and communicate this to the DON or Supervisor.
+ Works to establish a therapeutic relationship with residents, observing and maintaining their rights on an ongoing basis.
+ Participates in patient care conferences to develop, evaluate, and revise a plan of care.
+ Participates in staff development programs and unit meetings.
+ Channels administrative problems appropriately to DON or Supervisor, seeking supervision and guidance as needed.
+ Is aware of Presbyterian Homes Corporate Compliance policies including those having to do with confidentiality and security of information, and concern for the well being of residents and patients. Demonstrates behaviors and completes work in a manner consistent with these policies.
Education: Current Illinois license to practice as an LPN. Graduation from an approved Practical Nurse program. Completion of a pharmacology course.
Experience: At least two years as an LPN in a hospital (general care) or long-term care setting with adults.
Qualifications:
+ Good communication skills. Interest in working with older adults.
+ Ability to direct nursing assistants.
+ Knowledge of aging changes and common health problems of geriatric patients.
+ CPR certification.
+ Responsible to learn and use standard functions within software and know basic operation of computer to enhance efficiency in resident/patient service, assessment and/or clinical documentation. Responsible for using computers and software as required by work environment in order to enhance communications, performance and knowledge.
Physical Demands:
+ Regular bending, walking, lifting, or moving patients and/or equipment weighing over 50 pounds. (See attached.)
