How we want to change primary healthcare
- Evolve From Single Provider to Team-Based Approach: implement a team-based approach where multiple health professionals, including GPs, nurses, physiotherapists, OTs, psychologists, counselors, and health coaches (and more), collaborate to provide comprehensive care. Each team member practices at the top of their license, contributing their unique expertise to patient care.
- Integration of Technology and Health Services: Use cutting-edge technology to support care and cut out burdensome tasks. This includes developing unified health plans and records accessible and utilizing AI to assist in everyday tasks like documentation.
- Proactive and Preventive Care: Shift the focus from treating illness to preventing it. Position: Family Physician / General Practitioner
Location: Cordova Bay, Victoria BC

Overview:
As a Family Physician at Tall Tree, you will work with a team of engaged clinicians and orient your practice to proactive team-based health care. You’ll help usher in a new paradigm of care that focuses on bringing the best out of every health provider, and patients.

ESSENTIAL FUNCTIONS
- Conduct comprehensive health assessment
- Conduct complete reviews of patients health and history.
- Individualized Care Plan Development:
- Collaboratively design individualized long term health plans that offer complete options for patients in addressing nutrition, exercise, mental well-being, sleep, and all the dimensions of health.
- Interdisciplinary Collaboration:
- Work closely in a team of experts in physical health, mental health, diet & nutrition, exercise, sleep, behaviour change and more to provide a cohesive approach to patient care.
- Contribute to team cohesion and communication to ensure patients receive a seamless, integrated, team-based experience.
- Ongoing Patient Management and Support:
- Continually monitor, adjust, and refine patient care based on outcomes, feedback, updated research, and patient progress.
- Foster strong patient relationships, ensuring they feel supported, informed, and motivated throughout their journey.
- Deliver long term, longitudinal primary healthcare to patients.
- Educational Initiatives:
- Play a leading role in patient education, helping them understand their health and the importance of lifestyle.
- Participate in team training sessions to share knowledge, insights, and the latest research findings.
- Engage in outreach and community activities that inform the public about complete primary health.
- Program Advancement:
- Contribute to the evolution and improvement of care delivery by providing feedback, quality improvement, and staying up to date on the latest medical research.
- Collaborate with the management team to set goals, track outcomes, and ensure patient success and growth.
- Behavioral Change Implementation:
- Apply principles of behavior change to help patients implement and sustain healthy behaviors.
- Offer guidance on managing stress, substance use, and other lifestyle factors critical to improving overall health and extending healthspan.
Qualifications:
- Fully licensed (CPSBC) Practicing in Family Medicine
- Certification with the College of Family Physicians of Canada (CFPC)
- licentiate of the Medical Council of Canada (LMCC)
- Passionate about team-based and integrated health care.
- Strong communication and teamwork skills.
- Experience and interest in managing a diverse patient panel.
- Enthusiastic about using technology to enhance healthcare delivery
- Experience working with and leading interprofessional teams is an asset.
- Knowledge in preventive medicine & precision healthcare is valuable.
Funding and Patient Payment Model:
- Fee-for-service or LFP model
- Funding may be available through a provincially standardized New to Practice service contract developed by the Ministry of Health and administered by Island Health.
- There would be no cost to patients for access to primary health services with a Family Physician.
