Job Description
JOB SUMMARY
Under the direction of the Registered Nurse, provides total patient care in accordance with physician’s orders and individual patient needs in conformance with the recognized standards of nursing practice.
DUTIES AND ESSENTIAL FUNCTIONS
Overall performance rating of below indicated essential job duties:
Assesses patient and/or conducts screenings and documents utilizing standard scales when indicated as required per organization.
Prioritizes and documents patient problems based on assessed needs/problems in collaboration with patient, family, and health care team.
Documents changes in patient condition and effectiveness and timeliness of care/intervention.
Utilizes organization-wide safe medication practices including, but not limited to the 5 rights of medication administration, barcode scanning and other medication safety processes specific to the unit/department.
Knows actions and usual dosage, indications and side effects of medications administered.
Provides teaching and educational material to the patient and/or significant other as needed according to the care, treatment services and identified risks specific to the patient and documents patient and/or family understanding.
EDUCATION/LICENSE/CERTIFICATION/OTHER REQUIREMENTS
Minimum requirement:
Graduate from a Practical Nursing Program.
Maintains active LPN license in the Commonwealth of Kentucky (or other eNLC state with multistate licensure) and any other states as applicable.
Achieves and maintains American Heart Association Basic Life Support (BLS).

WORKING ENVIRONMENT
Constantly works near moving mechanical parts
Constantly exposed to blood and/or body fluids and infection.
Occasionally exposed to toxic or caustic chemicals
Occasionally exposed to risk of radiation
The noise level in the work environment is usually moderate.
PHYSICAL DEMANDS
Occasionally required to maintain in a stationary position (standing or sitting).
Frequently required to walk and move about.
Frequently required to operate a computer and other equipment.
Frequently required to use fine motor skills to handle and operate instruments/devices.
Constantly required to lift and/or move items up to 10 pounds.
Frequently required to lift and/or move items up to 50 pounds.
Occasionally required to lift and/or move items up to 100 pounds.
Constantly required to communicate with patients and other care team requiring talking and hearing.
Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, and depth perception.
Occasionally required to position self in order to meet patient needs.
