The Nurse Practitioner (NP), reports directly to the DOC and is also accountable to the Medical Director for meeting the Long-Term Care Home’s (LTCH) policies, procedures and protocols for medical services. The attending NP is a primary care provider to residents and works within her/his legislative scope of practice as described and outlined by the College of Nurses of Ontario. The attending NP collaborates with the resident and family/caregiver, and the health care team in the development, implementation and evaluation of the president’s plan of care; provides leadership and mentorship to the LTCH staff that enhances their knowledge, assessment skills, and ability to care for residents in place; and leads and collaborates in research, education, and evidence-based practice initiatives to optimize the resident, LTCH and health system outcomes.
Skills And Qualifications Required
- Current RN (EC) registration with a specialty certificate in Primary Health Care or Adult with the College of Nurse of Ontario.
- Minimum of 3 years nursing experience, ideally in seniors care.
- Nursing experience in an Emergency Department preferred.
- A valid Ontario driver’s license and access to a reliable vehicle.
- A passion for making a difference in the lives of our senior population.
- GNC(C) certification from the Canadian Nurses Association preferred.
Position Responsibilities
- Conducts incident investigations.
- Provides workers with written instructions as to the measures and procedures to be followed for their protection, ensuring every precaution reasonable in the circumstance is taken.
- Provide comprehensive resident assessment and treatment related to acute episodic and chronic conditions, following CNO NP Practice Standards, the CNA Framework for Advanced Nursing Practice (2008) and the PEPPA Framework (2004).
- Diagnose, initiate and manage the care of residents with diseases or disorders; palliative and end-of-life care; wound care and prevention programs (dehydration, falls, immunization); monitor, evaluate and document outcomes of decisions and interventions in a timely manner.
- Participate in strategic planning processes at management and program level; provide recommendations as necessary to Home administration to remediate nursing practice.
- Monitor and evaluate current practice at the individual and system level in the context of emerging research findings and best practices; disseminate best-practice innovations and recommend research-based policy changes.
