The Brockville Family Health Organization (FHO) is currently seeking a family physician to take over a well-established from a physician starting October 1, 2024.

The practice is located in The City of Brockville situated on the shores of the St. Lawrence River in the middle of the beautiful 1000 Islands region. The 1000 Islands region is a world-renowned tourist destination. In the summer, residents and tourists enjoy boating, fishing, sailing, camping, golfing, cycling, hiking, and scuba diving, among many other exciting activities. In winter, the area quiets down, but there is still lots to do, including cross-country ski, snowmobile, hike, ice fish, ice boat, skate, and play hockey. You can even enjoy downhill skiing just a short drive away. The region also plays host to year-round festivals and events, like the 1000 Islands Wine and Food Festival, the Tall Ships Challenge, and Brockville Ribfest.

This family practice is a well-established practice of 1100 patients and is co-located with 2 other physicians, administrative and nursing staff. This practice is part of a group of 15 physicians and is also a part of a larger family health team with many programs and allied health staff.

Brockville would be an excellent choice for a physician looking to fulfill Return of Service requirements as it is currently considered an area of high physician need (AHPN) with the Ministry of Health and Long-Term Care. The Brockville FHO is a blended capitation payment model, which includes an attractive base monthly rate plus Fee for Service percentage.

The Upper Canada Family Health Team (UCFHT) is composed of 26 physicians who work in groups of two to six over 9 different sites serving approximately 35,000 patients. The Brockville FHO is composed of 15 physicians spread over 7 sites in Brockville, serving approximately 24,500 patients. Working collaboratively within the team are allied health professionals and nurse practitioners providing primary care and programs. As a member of the Brockville FHO, you and your patients would have the support of various programs and services offered by the UCFHT, which include:

• Preventative Care
• Nurse Practitioner Acute Care Clinic
• Lung Health including Smoking Cessation
• Coordinated Care Plans / System Navigation
