Take Over Established FHO-FHT Practice on the East Hamilton Mountain

A FHO-FHT physician with a solo practice on the East Hamilton Mountain is seeking a successor to take over the practice in 2025 (start dates flexible). This opportunity offers the chance to work with a team of experienced administrative and allied health professionals in a supportive environment focused on team-based care.

With a roster of approximately 2,000 well-managed patients, the practice typically sees between 15-20 patients per day with appointments scheduled at 15-30 minute intervals depending on the complexity of each patient. Current practice hours are Monday - Thursday from 9am-5pm and Wednesday and Friday from 9am-1pm. Practicing within a large 24-member FHO, the practice provides on-call coverage to the FHO approximately 2 Thursday evenings every 3 months and 1 weekend every 24 weeks (with no nursing home calls and support from palliative care nurses).

As part of the Hamilton Family Health Team, the practice receives extensive support from a variety of allied health professionals including a registered nurse, dietician, clinical pharmacist, and mental health counsellor. Diabetic care is provided on-site by the clinical pharmacist and the registered dietician with supervision provided by the physician. The registered nurse provides extensive coverage on a variety of items including PAP smears, preventative care, prenatal visits, well baby visits, adult immunizations, ear syringes, stitch removal and admin support reviewing labs and consult notes, recording medications, and completion of some forms. The practice is also able to access to the HFHTs psychiatry team for urgent phone consults. The practice is also supported by 2 experienced administrative staff who know the patients well and handle appointment booking, patient screening, and reception.

The bright and modern practice uses Telus PS Suite EMR and includes 3 exam rooms, a counselling room, private staff washrooms, a staff kitchen, and a nursing station. Parking is free for staff and patients and the practice is located in a building with a radiology and pharmacy on-site and many shops and restaurants nearby. The practice is also conveniently located in a plaza just minutes from the Red Hill Valley Parkway and the Lincoln Alexander Parkway.
