Location: Midtown Toronto
As a new, private health clinic for women, we are seeking two energetic, full time RPN Patient Care Coordinators to join our midtown Toronto clinic.
As a RPN Patient Care Coordinator, you will foster a caring and supportive environment where patients will feel seen, heard, and empowered. You will be the personal health ‘concierge’ for patients during their time in the clinic. You will exhibit a passion for women’s wellness and be a key contributor to our inclusive culture and ongoing success.

Patients will have a seamless experience through our health offerings. Patients should feel welcome to give feedback to help us achieve our goals for continuous quality improvement. As a centre of excellence in women's health, you will confidently navigate feedback to ensure patients reach their personal health goals.
Your key responsibilities
- You will be a primary point of contact for patients, including telephone intakes and ensuring patients move seamlessly through in-person appointments.
- Provide current and prospective patients with information about clinic services and ensure patients are scheduled within the most appropriate program offering.
- Collect and document medical history in the EMR.
- Provide warm, personal service by guiding patients through testing procedures, room allocations, and offering assistance as needed throughout their time at the clinic.
- Serve as the follow-up contact for any questions or concerns after appointments.
- Work closely with clinic team members to develop and implement strategies that enhance patient experience.
- Provide patient-oriented care in accordance with the College of Nurses of Ontario standards or other regulatory body to which you are a member.
- Provide follow up and episodic care including by text, telephone, and in person, as directed by the Physicians and other health care providers.
- You will:
- warmly greet patients, take vitals, and establish a caring experience
- conduct phlebotomy
- prepare patients and rooms and assist with medical tests (for example, gynecological procedures, cardiac assessments/stress tests, DEXA)
- administer infusion medications
- communicate medical plans to patients as articulated by the Physicians and other health care providers
- Commitment to evidence-based practice and professional growth and development.
- Exhibit a passion for women’s wellness as they age.
A bit about you
- Registered and in good standing with the College of Nurses of Ontario.
- RPN accreditation with a minimum of 5 years of clinical nursing experience; experience in women’s health is an asset.
- Medical Assistants with a minimum of 5 years experience will be considered.
- International Medical Graduates will be considered if eligible to work in Canada.
- Proficiency with an EMR; experience with Telus CHR is an asset, training will be provided.
- Current CPR certification.
- Strong clinical assessment, critical thinking, and problem-solving skills.
- Excellent communication skills (both written and verbal).
- Excellent attention to detail.
- Exhibit a passion for making a difference in the lives of patients.

- Able to collaborate with other allied healthcare professionals to provide comprehensive and holistic patient care, demonstrating a team-oriented approach.

The annual salary for this role will be 65,000-70,000.
