ESSENTIAL FUNCTIONS:
Performs nursing care within the scope of education and ability, providing direct nursing care to residents.
Completes admission record, and initiates care plans.
Administers medications and treatments in accordance with established policies and procedures. Is familiar with indications of use, action, and major side effects of medications administered.
Takes direct action in urgent and emergent situations.
Anticipates and acts to maintain a safe environment.
Implements the plan of care for individual residents.
Supervises nursing assistant staff in direct resident care by:
1. Giving verbal and written report at the beginning of each shift.
2. Observing CNA/resident interaction and completion of resident care including bathing, grooming, dressing, feeding, toileting, mobility and attendance of activities, and appointments.
3. Discussing plan of care for individual resident.
4. Making rounds and receiving feedback on resident status at the end of the shift or at any time there is a change in a resident's condition.
5. Communicating information regarding job performance on an ongoing basis.
Evaluates, in collaboration with the Unit Nurse/Supervisor/Unit Manager, the job performance of CNAs.
Makes rounds on residents frequently throughout each shift observing for changes in condition and response to care with treatment. Is knowledgeable of common health problem of older adults and usual treatment, incorporating that knowledge into daily practice.
Documents appropriate and necessary information in the medical record.
Monitors the status of residents with acute problems and communicates this to the Unit Manager or Supervisor.
Works to establish a therapeutic relationship with residents on an ongoing basis, observing and maintaining their rights.
Participates in patient care conferences to develop, evaluate, and revise plan of care.
Participates in staff development programs and unit meetings.
Participates as assigned to a QAPI project
Channels administrative problems appropriately to Unit Manager or Supervisor, seeking supervision and guidance as needed.
Responsible to learn and use standard functions within software and know basic operation of computer to enhance efficiency in resident/patient service, assessment and/or clinical documentation. Responsible to use computers and software as required by work environment in order to enhance communication, performance and knowledge.
Completes all assigned computer education modules in a timely fashion
Assumes responsibility for own professional development
Is aware of Presbyterian Homes Corporate Compliance policies including those having to do with confidentiality and security of information, and concern for the well being of residents and patients. Demonstrates behaviors and completes work in a manner consistent with these policies.
EDUCATION: Graduation from an approved Practical Nurse program.
QUALIFICATIONS: Active, current Illinois license to practice as LPN.
At least two years as an LPN in a hospital (general care) or long-term care setting with adults.
Good communication skills. Interest in working with older adults. Ability
to direct nursing assistants. Knowledge of aging changes and common
health problems of geriatric patients. Current CPR certification.
