Job Description
Nurse Practitioners will provide care to individuals who need assistance as they deal with the effects of aging, injury, surgery, or illness. You will use an evidence-based approach to assess, complete a diagnostic work-up, treat, make referrals to specialists and allied health for patients and residents. Please note that this is a relocation position at a long-term care facility.
As a Nurse Practitioner You Will
- Help formulate a plan of care based on patient assessment in collaboration with the Care Team, facility, patient/family/caregiver and other health care professionals
- Perform advanced, comprehensive health assessments in-person
- Inform and establish comprehensive, holistic, therapeutic care plans using evidence-based practice
- Order and interpret diagnostics tests, prescribe medications, and send referrals to specialists
- Evaluate effectiveness of treatment plans and adjust accordingly in consultation with the patient/family/caregiver and other health care professionals
- Occasionally work autonomously and to full scope of practice
- Perform other clinical duties as required
Skills And Experience Required
- Excellent verbal and written communication skills to effectively interact with clients, colleagues, and other healthcare professionals
- Understand and respect economic, cultural, religious, racial, and gender diversity to provide inclusive and sensitive care to clients from various backgrounds
- Proficiency with computer and phone applications
- Experience in performing advanced, comprehensive health assessments in person or over a telemedicine platform
- Physical ability to stand, bend, walk, crouch, kneel and reach, ability to lift 50lbs (23 kg)
- Ability to interact with others in a caring, tactful and courteous manner
- Excellent problem solving, time management, and crisis resolution skills
- Exceptional customer service and ability to build a therapeutic relationship with the client
Education And Certifications Required
- Master of Nursing-Nurse Practitioner (MN-NP), Primary Health Nurse Practitioner (PHNCP), Master of Nursing/Primary Health Care Nurse Practitioner (MN/PHCNP), or related Nurse Practitioner Master’s Degree
- Current registration with the College of Nurses of Ontario (CNO) for Nurse Practitioner (Registered Nurses Extended Class) in good standing
- Valid Standard First Aid and Level C CPR
- Updated immunization records (including completion of a two-step tuberculosis test within the last 6 months)
- Clear criminal background check, and Vulnerable sector check are required upon hire
- Valid G2/G driver's license and access to a reliable car. Candidates without a vehicle will not be considered
Requirements
- Vulnerable Sector Check completed within the last six months
- Submission of proof of three COVID vaccinations
- TB test completed within 6 months to a year
- Immunization record
- Up-to-date CPR and First Aid certification
