Responsibilites
Substance Use Services Program
Collaborating with a compassionate and dedicated team of SUS Physicians, Physician Assistants and 24/7 Patient Care Coordinators (PCCs), we provide comprehensive intake assessments and long-term care to patients experiencing substance use concerns and/or substance use disorder. Our administrative support ensures a smooth and seamless experience for our patients.
- Medical management of all substance use disorders, including withdrawal management and anti-craving therapy
- Initial management of concurrent medical and psychiatric disorders
- Trauma-informed and patient-centred care
- Motivational interviewing
- Harm reduction education
- Infectious diseases screening
- Urine broad spectrum toxicology (BST) and/or ethyl glucuronide screening as appropriate
- Care plans integrated with existing virtual and in-person psychosocial and addiction services
- Participate actively in an on-call program
- Cross coverage of patients for urgent addiction medicine concerns, including withdrawal management
- Evaluation of patients at partner facilities, including emergency departments, inpatient units, withdrawal management services and shelters
- Communication with and transfer-of-care to family physicians upon stabilization
- Completion of Ministry of Transportation medical condition reports and substance use assessment forms
