Job Description
JOB SUMMARY
Plans, organizes, and conducts occupational therapy program in the patients home to facilitate development and rehabilitation of cognitively, physically, or emotionally disabled adolescent, geriatric and adult persons by performing the following duties under the general supervision of Coordinator of Occupational Therapist. Provides necessary supervision to COTA/L.
DUTIES AND ESSENTIAL FUNCTIONS
Overall performance rating of below indicated essential job duties:
Evaluates, assesses and establishes treatment plans on assigned patients in a timely manner and selects purposeful activities suited to the patient’s physical capacity, cognitive level, and interest to upgrade maximum independence, assist individuals in restoration of function, aid in adjustment to disability and assist in preparation to reach short and long term goals.
Collaborates with other members of the team to select the most appropriate activity programs consistent with needs and capabilities of the patient and facilitates the ability to improve functional, re-vocation and vocational homemaking sill and activities of daily living.
Re-evaluates effectiveness of interventions toward individual goals and makes revisions as needed. Evaluates need for treatment discontinuation or discharge and recommends appropriate continuum of care when maximal potential has been achieved.
Instructs compensation techniques and designs and fabricates orthotic devices and assistive/adaptive equipment for the patient and suggests adaptations to his/her work/living environment. Instructs patient and family in the treatments to be continued at home.
Supervises patient treatments to the COTA/L and follows progress updating goals as necessary.
Documents assessment, plan, treatment and goals in EPIC.
Collaborates with other members of the healthcare team to identify needs and continued services once discharged from current level of care.
EDUCATION/LICENSE/CERTIFICATION/OTHER REQUIREMENTS
Minimum requirement:
Bachelor’s Degree in Occupational Therapy
Maintains active licensure in the State of Kentucky and Ohio as an Occupational Therapist
Achieves and maintains American Heart Association Basic Life Support (BLS)
Maintains active driver's license.

Constantly required to lift and/or move items up to 10 pounds.
Frequently required to lift and/or move items up to 50 pounds.
Occasionally required to lift and/or move items up to 100 pounds.
Constantly required to communicate with patients and other care team requiring talking and hearing.
Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, and depth perception.
Occasionally required to position self in order to meet patient needs.
