We have been in operation for over 15 years and are looking for a highly passionate Nurse Practitioner to join our very dedicated team of providers.
Healthcare is changing very quickly and the role of prevention is becoming more and more evident. Preventous recognizes and values the importance of the Nurse Practitioner role in healthcare due to the combination of medical skill and the caring aspect of a nurse.
AN IMPORTANT ROLE
Your role will be an integral part of our model and will encompass co-ordinating and delivering patient care in family medicine. You will be required to assess and manage patients with a focus on prevention and optimizing health. You will collaborate with our team of healthcare providers including our physicians, dieticians, kinesiologists and athletic therapist to formulate health plans and follow through with such plan, including referrals, counselling and treatment.
Day-to-day responsibilities include:
- Co-ordination of patient care
- Patient intake and triage
- Diagnosing illness
- Performing Total Health Assessments and Reviews
- Ordering laboratory tests
- Counselling patients on mental health
- Counselling patients on lifestyle habits and prevention
- Prescribing medication
- Assessing patient and creating plans in follow up to assessments
- Making referrals and coordination of diagnostic testing in collaboration with family physicians
- Coordinating care with dietitians and kinesiologists
Qualifications
- Masters of Nursing - Nurse Practitioner, post-masters diploma
- CARNA registration in good standing
- CPR
- Ability to treat a diverse population - adults and children
- Knowledge and experience in primary care
- Current experience in chronic disease management preferred
- Advanced clinical nursing skills within the scope of a FNP; additional training relevant to practice is an asset
- Experience in mental health is an asset
- Skilled in program planning, implementation and evaluation
- Ability to take initiative and demonstrate proactive/preventative care
- Exceptional organizational and personal skills, positive and results focused attitude, and excellent written and oral communication skills
- Ability to work in a collaborative setting
- Computer competency and familiarity with an EMR
- Experience with medical
