Work Location Arrangement: Fully On-site the position requires 100% on-site placement; it is operationally dependent based on established criteria; remote work is not an option.
Salary: $53.89 $61.84 per hour (as per ONA collective agreement)
Position summary:
We are currently seeking a highly motivated health professional to contribute to our organizational mission of working with our communities to promote and protect health and prevent disease.
Reporting to the Program Manager, the Nurse Practitioner develops, implements and evaluates health care programs, assignments and/or services for individuals, families, groups and the community in accordance with the divisions mission, philosophy and program/operational plans.
The Nurse Practitioner functions as a primary health care provider by acting as consultant, educator, facilitator, advocate, counsellor, health promoter, social marketer and community developer. The nurse practitioner will work with public health nurses to deliver the full scope of clinical services throughout the agency.
Responsibilities:
- Provides orientation and preceptorship experiences for students and multi-disciplinary staff as required.
- Acts as a resource person to the organization and the community.
- Provides input for the ongoing development of the divisions philosophy, goals, objectives and program/service activities.
- Participates in division and team in-services, and contributes to division staff development needs.
- Performs comprehensive and holistic health assessments, and synthesizes data from multiple sources to make a diagnosis within the scope of practice of the RN (EC).
- Provides direct nursing care and counselling on health related issues.
- Uses clinical skills to assess the clients health status to enable joint planning, implementation and evaluation of appropriate nursing interventions.
- Initiates, manages, directs and monitors the care of clients, based on the assessment findings, within the scope of practice of the RN (EC).
- Implements strategies to promote health with clients and within communities, and to prevent injury/illness with clients.
- Performs clinical procedures as within scope of practice.
- Prescribes and dispenses medication as appropriate within scope of practice and in accordance with College of Nurses standards and agency policies and procedures.
- Performs case management in accordance with program requirements.
- Acts as a resource and support to staff in the clinic and to other divisions of the agency.
- Maintains documentation records according to the standards of the College of Nurses and provides reports. Writes articles for newsletters, newspapers and other media.
- Represents the department, division and program on community initiatives committees and coalitions providing consultation and exchange of information.
- Uses expertise in policy advocacy to support political action initiatives within the framework of an approved agency plan.
- Assists with the design and development of educational resources in accordance with agency media communication policies.
- Handles complaints/problems and difficult situations appropriately.
- Performs other duties as assigned.
- May participate as directed in enforcement of Health Protection and Promotion Act.
- Identifies supplies, equipment and resources necessary to carry out programs in support of the divisional budget preparation process.
- Conducts projects in a fiscally responsible fashion.
- Contributes to the development of program plans and develops, plans, implements, coordinates and evaluates assigned projects in consultation with the manager and other team members as required.
- Participates in the orientation of staff, students, volunteers and others as may be required for the position.
- Provides input into the budget process upon request.
Requirements:
- Proficiency in both official languages is required.
- Recognized Bachelor of Science in Nursing with Registered Nurse, Extended Class registration from the College of Nurses of Ontario.
- Current certificate of competence with the College of Nurses of Ontario.
- Incumbent must have current certification in the following prior to start (at their own cost):
- CPR Health Care Provider/Basic Life Support + AED
- Training on the administering of Naloxone will be provided.
- Maintains and demonstrates professional competence as a Registered Nurse Extended Class and is accountable for professional nursing practice in accordance with the College of Nurses Standards of Practice, other relevant legislation and health unit policy.
- Experience working with and providing services to people experiencing mental health challenges, mental illness, substance use, and/or addiction.
- Knowledge of best practices and guidelines on sexually transmitted and blood-borne infections.
- Demonstrated ability to utilize the nursing process, assess the health-related needs of individuals, families and groups and the community, manage time effectively, mobilize and facilitate groups, lead, and co-ordinate.
- Well-developed decision-making, problem-solving, interpersonal, and organizational skills, including the ability to work both independently and on a team.
- Demonstrated computer skills and the ability to analyze and interpret computer spreadsheets and research databases.
- Knowledge of current practice in relation to ethical and legal issues.
- Excellent verbal and written communication skills.
- Demonstrated ability to work with diverse populations in a respectful and non-judgemental manner.
- Ability to work flexible hours as detailed in the collective agreement.
- Must have a personal cell phone (smart phone) with Internet access and be willing to download an application to confirm identity via two-factor authorization.
- Satisfactory police reference check in accordance with agency policy.
- Current Ontario drivers licence and access to a reliable vehicle.
- Ministry of Labour Worker Health and Safety Awareness in 4 Steps training certificate is required.
