Responsibilities:
- Review 24 shift report after coming on to your shift.
- Run the report for Skin and Wound issues.
- Do rounds on all units and ask the unit nurses if further follow up is needed for the worsening wounds.
- Complete the skin and wound assessment for all the floors as required by home procedure.
- Complete the skin and wound audit weekly and follow upon deficiencies.
- Ensure all floors have required supplies for skin and wound.
- Do rounds with the ET nurse monthly and follow up with her recommendations.
- If there are new orders for supply, contact the management.
- Communicate with ET nurse re: her rounds date and send her the list of residents to be seen for her visit.
- Review UDA’s and complete them.
- Review Risk Management.
- Ensure all steps and assessments and follow up are completed as per Extendicare policy.
- Orientate the newly hired staff re: program.
- Conduct Monthly Skin and Wound and Continence Committee meetings.
- Complete the Annual Program Evaluation for Skin and Wound.
- Insert the new monthly tracking sheets at the end of the month for all floors and collect the old ones and submit to ADOC /DOC.
- Review the 24hr infection surveillance daily to identify the skin and wound infections.
- Review and update care plan as needed for skin issues.
- Participate in team/committee meetings.
- Start/complete mandatory online training (e.g., AIS, SURGE, 24-7).
- Conduct rounds after review of 24-hour report.
- Follow up with MD/NP as needed.
- Assess and distribute supplies.
- Orienting, mentoring, and supervising new staff (PSW, RN, RPN, students).
- Carry out assigned duties in compliance with applicable OHS legislation and Company policy.
- Assist RPN with any clinical procedures (e.g catheterization, hypodermoclysis etc).
- Help the RPN on physician day with medication order processing or calling the family.
- Every 7th of each month complete the weights Audit and submit it to ADOC/DOC.
- Spring/Summer Months-lock all patio doors and receiving door by 9:00pm.
- Other follow ups as assigned.
