The Nurse Led Outreach Team (NLOT) is a team of specialized nurses who provide Mobile Nursing Services to the residents of Long-Term Care Homes (LTCHs), contribute to LTCH capacity building and optimize access to services. The Nurse Practitioner (NP) having a specialty-based focus functions in an expanded role demonstrating a high level of autonomy and expert skill to formulate clinical decisions and appropriately manage acute/chronic illness and promote wellness. As a member of the Interprofessional Practice team, the NP will work in partnership with the Geriatric Program to utilize clinical expertise and an in-depth knowledge of senior’s patients in Long Term Care Homes within the South Georgian Bay.
The Nurse Practitioner will network to establish relationships with partner Long Term Care (LTC) Homes and staff, community organizations, physicians, and local hospital staff to enhance the circle of care for complex residents and their families. In addition, the NP will provide clinical leadership and promote a culture of excellence within the assigned program and across the organization. The NP will do this by providing expert clinical practice, education, consultation, treatment, research within a family-centered nursing approach.
Responsibilities Include:
- Conducts in-depth comprehensive assessments of LTC residents that integrate a wide range of complex information including resident, family, and staff perspectives, nursing allied health theory
- Initiates and evaluates selected medical and nursing therapies within the NP/RN scope of practice, medical directives, and practice guidelines.
- Utilizes diagnostic reasoning to formulate a medical diagnosis within the specialty scope of practice and consistent with practice guidelines.
- Collaborates with the physician when determining an acute or chronic diagnosis as appropriate.
- Consults with and refers to other health care professionals and community services as needed.
- Provide education informally and formally to internal and external teams/partners.
- Participate in program evaluation, reporting requirements, and quality improvement initiatives as required.
- Performs other duties as required or assigned.
Qualifications:
Education/Certification:
- Master in Nursing/Nurse Practitioner (MN-NP) or Masters of Science in Nursing with successful completion of a post Master's Acute Care Nurse Practitioner Certificate Program from a recognized school of graduate studies required.
- Current College of Nurses of Ontario registration in the Extended Class (EC) as an NP (adult) or NP (primary health care) required.
- Current certification in Basic Cardiac Life Support required.
Experience:
- Minimum three (3) years clinical experience in geriatrics required.
- Advanced knowledge of the nursing process, the consultation process, program planning, nursing research methodology, discharge planning, and community resources required.
- Eligibility for cross-appointment to an academic institution preferred.

- Satisfactory criminal reference and vulnerable sector screening prior to hiring.
- Valid driver’s license required.
