WHAT YOU’LL DO:
Reporting to the Clinical Lead, Virtual Care, the role of the Nurse Practitioner is to:
- Assess and evaluate acute and chronic health conditions for patients with an emphasis on assessment, diagnosis, treatment, follow-up, counselling, screening, education and referral.
- Provide health education and counselling focused on health promotion and disease prevention.
- Improving patient outcomes along the continuum of care through utilization and application of up to date evidence based guidelines, interdisciplinary collaboration, and participation in research.
- Work with the Clinical Lead and Virtual Care Team in developing specific policies and ongoing education to take action on patient virtual care needs.
- Assist in the development and evaluation of protocols and Quality Improvement in order to provide optimal standardized care to clients.
- Provide leadership to promote program development, process improvements and the enhancement of professional standards within the clinic or virtual care platform.
- Act as a clinical resource for nurses and other health care professionals.
- Work collaboratively with patients, physicians and other members of the interdisciplinary team to develop a plan of care.
- Articulate and demonstrates the integration of the NP-PHC role through research, education and consultation.
- Additional duties needed as per the scope of the NP practice and needs of the patient population and organization.
WHAT YOU’LL BRING:
- Registration without restrictions as a Primary Health Care Nurse Practitioner through your provincial college (BC & ON mandatory, multiple licensures is preferred).
- At least 3-5 years as a Nurse Practitioner providing care on a virtual and/or in person clinic.
- Experience in urgent care an asset.
- Sound clinical judgment and strong health assessment and diagnostic skills.
- Demonstrated ability to work independently and as a member of a multi-disciplinary team.
- Superior and Effective communication, interpersonal, organizational and administrative skills, including computer literacy skills specific to job requirements.
- Willingness to teach /work with other professional disciplines.
- Candidate should be able/willing to work any days of the week when hours have been determined and open to different shifts.
