The Owen Sound Family Health Team (OSFHT) is committed to improving and maintaining the health of the patients we serve through a team of family doctors, nurse practitioners and other health care providers. Based on local health and community needs, we provide access to innovative, high quality and comprehensive primary health care programs and services that are based on the needs of the patients we serve.
The Programs & Services RPN works collaboratively with a variety of healthcare professionals to facilitate the delivery of nurse practitioner primary care services. The RPN will work within their full scope of practice as defined by the College of Nurses of Ontario. New graduates are welcome to apply.
Accountability
The Registered Practical Nurse reports to, and is responsible to, the Executive Director and the Programs & Services Manager
Roles and Responsibilities (but not limited to)
- Medication administration incl. routine vaccinations and injections
- Monitoring vital signs incl. blood pressure, HR, O2
- Supporting physicians with routine Diabetic care appointments.
- Supporting OTN visits
- Providing care co-ordination with CCAC, Public Health, etc. regarding patient care
- Communication with patients
- Management of medical supplies, and infection control procedures
- Production and management of patient health information packages
- Other duties are required by the organization
Qualifications
- Current registration with the College of Nurses of Ontario in good standing required,
- Current registration with the RPNAO with Professional Liability Protection (PLP)
- Knowledge of requirements of the College of Nurses of Ontario, privacy legislation, Regulated Health Professions Act, 1991, and Scope of Practice as per RHPA: Scope of Practice, Controlled Acts Model, 2011 for nursing
- Current CPR, Mask Fit and Vulnerable Sector Police Screening required
- Previous clinical experience in a primary care setting or Family Health Team is an asset.
- Excellent interpersonal skills re: patients and their families, demonstrating the ability to emphasize the interpersonal aspect of nursing such as interest, empathy, honesty, non-judgmental attitude, and respect
- Must be team focused and flexible; works collaboratively with the multidisciplinary team
- Demonstrates excellent problem-solving and organizational skills, as well as written and verbal communication skills
- Ability to multi-task, work under pressure, and meet deadlines while making ethically sound decisions to ensure safe patient care
- Proficient computer skills and effectively use an Electronic Medical Records (EMR) systems for clinical documentation. TELUS PSS experience would be an asset.
- Excellent work performance and attendance record is essential

Salary: $25.00-$27.26 per hour
