Reporting to the Operational Director, Clinical Support Services and under the direction of the Pharmacy Professional Practice Leader, the Pharmacist will provide consultative and professional pharmaceutical services, and will assist in co-coordinating the activities of the Pharmacy department. The pharmacist participates in and provides oversight for drug distribution activities, and shall ensure that drugs are procured, stored, prepared, prescribed, dispensed and administered in a safe and effective manner. The Pharmacist will also provide clinical pharmacy services such as comprehensive drug information used in the care of St. Mary’s of the Lake Hospital clients, including promotion of appropriate pharmacotherapy to optimize patient/client outcomes. The Pharmacist will participate in education activities related to their area of practice. Working with the clinical, Nursing and Allied Health Care services staff, the Pharmacist will provide pharmaceutical care to clients across the continuum of care.
Skills And Qualifications Required
- Bachelor of Science in Pharmacy, or PharmD
- Current, unrestricted registration with the Ontario College of Pharmacists
- Successful completion of a Pharmacy Residency Program accredited by the Canadian Hospital Pharmacy Residency Board, preferred
- Minimum of three years of experience in hospital pharmacy practice, preferred
Position Responsibilities
- Perform therapeutic review of client medication orders and prescriptions to ensure completeness, accuracy and appropriateness
- Identify and communicate actual and potential drug related problems to pharmacist or physicians as appropriate
- Perform final check on new client medication orders and prescriptions
- Supervise Pharmacy Assistants and Pharmacy Technicians in the provision of drug distribution as it relates to receiving, preparing, checking and dispensing drugs
- Function as a clinical pharmacist in the provision of direct client care
- Identify, resolve and prevent drug-related problems using a client-centered, collaborative approach
