Summary of Position
Calgary Foothills Primary Care Network (“PCN”) is a group of family physicians and health care professionals in northwest Calgary and Cochrane, collaborating with Alberta Health Services – Calgary Zone to deliver the best possible primary care. Calgary Foothills PCN has established a team environment where individuals are encouraged to take initiative, be creative and contribute to ongoing decision-making.
This position will support two programs that provide services to patients with complex chronic health conditions out of the Crowfoot Primary Care Centre.
The Extended Health Team (EHT) is a comprehensive interdisciplinary team caring for patients by referral from the member medical homes. The EHT helps patients with complex chronic health conditions who struggle to self-manage chronic conditions, set achievable goals, and access additional resources and support. Group and individual appointments or home visits are tailored to the patient’s needs.
Key Responsibilities
Key aspects of the position may include, but are not limited to:
Patient Care
- Perform patient health assessments and health maintenance activities (e.g., history and physical, wellness exams, pap tests and breast exams, prenatal care, well baby visits and immunizations)
- Monitor the ongoing therapy of patients with chronic stable illnesses by providing pharmacological and counselling interventions
- Screen for the presence of chronic illnesses
- Conduct examinations using an age/sex specific list of recommended preventative interventions and risk assessment
- Diagnose and treat acute/episodic minor illnesses (e.g., respiratory illnesses, ear and throat, gastrointestinal, genital urinary infections and dermatology) and injuries
- Offer phone consultation/triage for acute episodic illness
- Formulate and communicate medical diagnoses and disorders
- Determine the need for, order and interpret X-Rays, ECG’s and diagnostic ultrasound
- Prescribe drugs from an approved list
Referral/Consultation/Collaboration
- Consult with physicians in accordance with the standards for physician consultation
- Liaise with other team members as required
- Arrange for external specialist consultations
- Consult with hospital and community health care providers to ensure seamless care transitions
Administration
- Develop, initiate, and maintain a preventative health monitoring program using the Electronic Medical Record (EMR) and working with other health care team providers (e.g., monitoring for cholesterol tests for men and women over age 45, pap tests for all sexually active women between ages 20-70, blood pressure monitoring for men and women over 50 years)
- Arrange follow-up appointments as necessary

- Ability to provide evidence-based care for patients with chronic diseases

- Advanced understanding of computer skills are required
- Strong health assessment and documentation skills
- High degree of accuracy and attention to detail
Qualifications
- Bachelor’s degree in nursing
- Master’s degree in advanced nursing practice
- Licensed with the College and Association of Registered Nurses of Alberta as a practicing member, with Family/All Ages (FAA) registration
- Advanced experience and knowledge in Geriatrics and / or Chronic Pain Self Management required
